Oops. Just completed mine, though I doubt not filling out the transport form results in no entry. It stops shy of saying that specifically.

You should also have had an email titled "Action Required – Transport Options" that had the info, and they also needed you to complete the travel survey to confirm your entry.
Deadline for that was 15/07/16 so I'd def give them a call
