A new tube is a guaranteed repair, but I carry patches too. I have a mini pump because I know I only need to get to a bike shop to top off. Also useful to be able to put some air in to find the puncture. I'd carry CO2 and mini pump if I was in the middle of nowhere, unless I was confident the pump would get the pressure I need.
As for which, I have a lezyne and a Zefal HPX. Some of my bikes have mounts for the frame pump, but the lezyne lives in my saddle bag. Watch out for removable valve cores, the lezyne can unscrew them!
