In the past I have put as much pressure on the extractor tool as I darewith the bike on its side. Then the fun bit.....if you can keep the pressure on whilst you slowly pour a hot/boiling kettle of water on it. Or get someone else to do either the tensioning or the pouring. It might then pop off for you into you blistered hands. Alternative sources of heat might also work but less exciting.

Got a square taper NDS crank stuck. Really doesn't want to shift.
Have you stripped the extractor threads yet? If not, you're not trying hard enough :-)
I've yet to see a crank which can resist my gear puller/bearing splitter, this pattern is best for cranks but the ones with two or three fingers have been used successfully by other people, although they tend to mark the back of the crank more.
