I use Lastpass on computer and phone. It's a browser add-in so works fine on Windows or Linux which I use it on.
The mobile version is $10-$15 per year. You can log in and view/copy usernames and passwords or it autofills into the browser and apps (although it's a bit erratic as to when that works).
There is a web version, although as I have two factor authentication turned on I need my phone to log in anyway so I don't use that.

Also a couple of questions re Lastpass. I've been idly reading around as I've had a few scares this year with a couple of low level hacks of accounts that have similar passwords to other accounts. There seems to be a lot of grumbling about lastpass being acquired by/merged into logmein, is this just nerdrage or are there genuine security concerns? Also, after all my passwords have been super secured up how easy is it to log into stuff when you are away from your main computer? Can you look at them from a phone app or is there some kind of cloud based log in system? Can you carry lastpass round on a usb key or suchlike?
