where to get some of these machined?
Your friendly local CNC shop, pick a few from google and go round and see who seems friendliest. Price quotes can easily vary 3:1 depending on whether they actually want to do them, but even the cheapest is likely to be off-putting if you only need one set.

Any reason for clamp-on, rather than having a local framebuilder braze some mounts on directly?
