It's a "Planet X Superlight Team 3D Forged Stem": 80mm/17°/31.8mm/1 1/8".
Bought at the beginning of this year for my TT bike, the only blemishes are a couple of rub marks from the top cap (see last photo).
New from PX will cost you £19.99 plus £3.95 postage; second hand from me will cost you £10 plus £3.90 postage.
