I have often fancied being a paramedic.
Last week I was marshalling a running race and got chatting to one. He said it's pretty common for people to do a mid-life career change into the service, but he'd advise against working for the NHS.

I have some old-ish Mavic Reflex Clincher rims. Internal width 13 mm.

My bike has had the steerer tube extended at some point, it only allows for around 6-7cm of quill stem insertion before it hits a metal sleeve inside the steerer (I assume to strengthen the join between old and extended steerer?)
I can only fit a stem in fully if I cut the bottom of the quill stem off.
- is this a safe amount of stem insertion and is chopping the stem dangerous?
- normally the stem bolt isn't threaded high enough to tighten the short stem. this has left me stuck with a crappy old scrap yard stem. I want to replace it.
Are there any ways around this?

Any barrel adjusters that don't slip and undo themselves? Any solutions to stop this? Loctite/plumbers tape? I'm using jagwire ones at the moment and I'm finding with the high tension needed for fd-6800 it slowly unwinds itself over 2 weeks of use or so.

Could easily just be the cable housing need compressing.
Start again, set adjuster to zero, then make sure cable really tight first time round.

I am a member of lfgss cc. Not really for any good reason other than I like it here.
I don't race. My cycling is generally commuting or audaxes.
I am about to join another London club. They have proper membership and "second claim" membership with quite a difference in prices. I am more likely to go on club runs with the new club as they ride multiple times a week.
I should join them properly shouldn't I and have lfgss cc as a second claim?

As I understand it, if you're not racing then it really doesn't matter how many cycling clubs you join. The concept of first and second claim is purely related to racing.

Seeking advices on pedals... Pretty much set on Shimano SPD-SL.
I have trouble seeing the difference between Ultegra, 105-c and R550, except a weight difference. Is there anything I am missing ? -

No, all Shimano pedals (with the possible exception of very cheap OEM MTB pedals) work equally well. If you were a weight weenie, you'd be using Keos anyway :-)

I've got rental vans despite having a different address on my licence, but the counter staff can be dicks about it. It would help if you've kept any utility bills from the old address, but again they tend to be dicks about anything more than 3 months old.

Then you need the 'proof of address' shit like utility bills for that address. Rental companies differ, but sometimes it seem like having a forrin passport is the easiest way to get a vehicle as they then don't ask for any other ID.
