Yeah it's dead easy even with basic sewing skills.
Turn jeans inside out, cut out pockets (the normal fabric bit, not the jeans bit) trace on new fabric, stitch together, sew back on. Keep in mind that a thinner but still durable fabric is best, anything too sturdy might get uncomfortable.
