I think it's the extended adapter not sure it applies to the 3-bolt. It's in the instructions.
If you're getting cleats they should come with whatever you need.

Everything you need for 3 bolt is included with the pedals, including a couple of different bolt lengths. Adaptor plate adds a couple of mm to stack tops.

As I said above, the sigma pedals come with v2 cleats

Had an e-mail from Sigma: the C-M are c/w V2 cleats too.

Where can I get cleat screws - the little ones, not the wide adapter ones (without paying an arm and a leg)?

I measured one of the ones I haven't lost at 11mm, and ordered the 11mm ones here: http://www.wiggle.co.uk/speedplay-screw-kits/?sku=5360348694 which look like the photo, and received the wide adapter ones instead.

Arf, just seen this on speedplay's site:

While V.2 Cleat fastening screws are required for V.2 Cleats, the new V.2 Cleat fastening screws can be used with all pre-V.2 cleats and base plates.

So, semi threaded V2 screws should work with old cleats.

Edit: confirmed, V2 screws fit fine in old cleats.
