Wow, it's a monster!
Le Manfriend built himself a Long John style cargobike, but it does have a 5 speed SA gear (and drumbrakes for that nice hill he lives up...) it may indeed be very hard to use in non-flat areas w/o gear.
Going to be awesome though with a nice box/some TLC.
