  • took a week off the bike and knee was loads better, cycled 15 miles or so over a couple of days and can start to feel pain again.

    Whats odd is that I can feel the pain start even though I am on my fixed gear with straps and not limited in position so much as my road bike with cleats.

    Basically if I sit on a chair and then twist my right leg down towards the floor I get a fairly sharp pain on the inside of my knee, quite hard to locate.

    Like I said - its not a sharp pain all the time or even terribly bad but the more I cycle the more it seems to aggravate it

  • Is it possible to get knee pain from cycling when all i'm doing is 4 miles each way a day (approx 5 hrs a week cycling)? my right under-knee is really painful when doing anything (stairs, standing up) and the only exercise i do is cycling but just commuting atm.

  • yes - as said in my post just above, I cycled a mere 15 miles this week and can feel my knee a bit. Youre cycling what 50miles? thats a reasonable amount imo.

  • must be down to my cleat position (road shoes) absolutely cannot afford a fit though..

  • I'm still suffering with the itb, and I'm only doing about 6 hours a week atm. It's a thousand times better since using the foam roller (I can go down stairs without swinging off the banister) and I'm trying to stretch it 3 times a day, reckon it should be 100% in 3-4 weeks time.

    I've slight knee pain around the patella, top and inside area, for a couple of weeks I've rested the leg but it's still there somewhat.
    Has anyone positive experience of asking their GP to look at knee pain? Got the feeling I'll go there and they'll type what I describe into Google and tell me to rest it.

  • At least you can get a referral to a NHS physio?

  • Don't let them fool you into painkillers and rest, they did it to me. Just sit there and insist until you get an appointment with an specialist. That said the physio would take half an hour weekly with you at best, so if you have a bad case an money to burn it may be better to go private.
    In the meantime try to some exercises for strengthen it

  • I've developed right knee pain since switching bikes (technically my summer bike). I think it's my ITB and it became so painful during my ride this morning that I had to turn back 20 mins in. I don't really understand where it has all come from since both my road bikes have the same pedal setup and I've been wearing the same pair of shoes (so I presume cleat positioning isn't the issue?). Also, I've not been over-training and I don't have any flexibility issues that I'm aware of. It seems like the bike switch has precipitated the issue so perhaps I need to play around with the saddle height and setback on the summer bike?

  • Have you had a spirit level and tape measure out to confirm your set up is identical?
    Same cranks? Q factor? Saddle setback? Bar reach? Stack height?

    Anything else is guesswork.

  • I'm getting bit of knee pain recently. I think (hope) it's down to the colder weather but could be coincidental I guess or colder weather showing up an underlying problem?

    Never seem to feel it on the commute (4 ½ mile each way on a variety of bikes using Atacs and flats) but havn't had any very cold commutes yet this year.

    Have felt it when riding ss mtb with Atacs (actually this wasn't in cold weather), felt it a couple weeks ago riding my Surly (geared/Atacs) in one pair of shoes and again today on that bike but in a different pair of shoes. I haven't taken any steps to ensure cleat setup is the same between shoes other than bolting them on and checking they feel comfy. I presumed increased float in the Atacs would accommodate any errors in clear position.

    Didn't feel it yesterday riding a different bike with flat pedals.

    Seems to be around 6-10 miles into a ride I'll feel it.

    Today it came on while I was riding along a flat canal path and with no change to terrain etc I found it had disappeared a little while later. It came and went but notably wasn't present while grinding up an off-road climb a bit later, air temp was pretty similar but could there have been more warmth in the joint by this point? Came back with a vengeance after a fast very cold descent where I'd been mainly coasting so the knee hadn't been moving.

    Once home and warmed up there was no residual pain or anything. Bit of pain if I kneel or go on hands and knees just now actually.

    been to the physio twice, have discovered that my right leg with the bad knee constantly wants to twist outwards.

    If i sit and dangle my legs off the edge of something my right foot naturally twists outwards.

    Anyway, it would appear that the primary cause is my hip rotator but it is also affecting my ankle and thus I also have poor stability on the right side when trying to do a single legged squat.

    its taken 2 hours to get to this conclusion, but i am happy thus far. It was especially impressive due to the fact that no matter how she twisted and contorted me we were unable to replicate the same pain, other than when I sat down and twisted knee outwards. Bizzare.

    Anyway its certainly not fixed but I hope that it is something I can now work on. Im glad there is now something i can visibly see (foot twisting outwards).

    Going to go for a cycle tomorrow and make a conscious effort to keep my foot in a straight line and see how my knee feels after that.

    tldr - its most certainly not always JUST your knee.

  • It would be amazing if just being mindful of your knee is all it takes. The problem is when you get tired or have to focus on something else you are likely to revert to your past form. Hopefully the Physio will have suggested some exercises to strengthen and stabilise the knee, and possibly some stretches too.

    It sounds like cleat alignment, wedges and possibly orthotic insoles may help. A Physio who works with cyclists a lot or a bike fitter may be able to help with this.

  • yes of course, I have a whole bunch of stretches and exercises to do to loosen up one side and strengthen the other.

    My point was that provided I am aware of tracking my knee I should be able to cycle in the mean time. But I have yet to go out, so we shall see.

  • I've had maybe a few months without much riding but managed to get out a bit over Christmas. First ride was 60 miles and the top of my knees gave me some serious trouble for the next two days. The last three or four rides have been shorter as I'm worried about aggravating my knees again. I was regularly doing longer distances over summer with no knee issues.

    The only thing that has changed with my set-up is that I've been wearing tights lately. Could the pressure from the tights really be causing an issue?

  • Not riding much and then doing 60 miles was probably a bit of a stress to your knees.

  • Out on flat pedals today and got some knee pain. I guess that means it's not a cleat position problem.

    Wonder if it could be to do with riding the derny at work as I ride it with the prescribed toes out position to create a bigger draught. Might no longer be an issue as I've not renewed my BC membership so might not be eligible to ride them anymore.

  • Paging both @scherrit and @bluerip00 on this one as I'm not sure if this is simply a muscle imbalance issue or a bit of fit too.

    I've been experiencing a bit of knee pain over the last couple of weeks. It is on the inside of the left knee and builds over the course of about 2 hours riding and disappears after about two days rest. This has happened twice now and each time I've tested the knee on the turbo to check all is well before hitting the road again. Both times I've had no pain on the turbo (60-90 mins at tempo/threshold), but the pain has come back on the road.

    I've just finished another hour on the turbo after letting the knee recover over the last few days and I've made some observations.

    My knee tracks perfectly when putting down power. Tempo and upwards the mechanics look to be fine. As soon as I lift off I can see my knee flick out slightly at the very top of the pedal stroke unless I really concentrate on keeping everything in line. Back on the power and tracking is good once again.

    I saw @bluerip00 a while back about an ITB issue on my other knee that was a result of muscle imbalance. The strengthening exercises worked and the pain went. I'm assuming this is a similar problem and have started the same strengthening and stretching programme as last time in the hope it'll prove as effective. We will see.

    If anyone can offer any advice or help explain why this is happening when soft pedalling, but not under power, I'd appreciate any help.

  • I have BUPA full cover through work, but Ive never used it does anyone know how I should start the process of seeing someone about knee pain?

  • been for a few rides, kept leg/knee inline and seemed fine. Another longer ride (still tracking knee movements) and pain was back. Week off and knee feeling totally fine. I then went for a short 10 on the turbo, keeping eye on knee tracking and pain is back.

    totally gutted, losing weeks and weeks to this :(

    throwing money at it, yet not 100% convinced what the issue is.

  • Sorry to hear this. Hope you get it sorted. Any luck with cleat wedges/insoles/different length pedal spindles?
    Are your legs same length?
    Best wishes to all fellow sufferers. Had a few twinges since I've gotten new shoes and need to spend money on insoles and bits making them like the old ones...
