Keep investigating brake pads, for certain rims pads can be a real issue. My commuter bike the rim is a mavic MA40 and it seems to chunk off into the brake pads every few days = no braking + huge gouges dug into the surface of the rim.
Changed to a different pad (was shimano XT canti pads) and problem solved.
