If you're going to run a lower gear to get rid of you knee problems then perhaps running a larger one again will just bring them back? Just a though.

19/48t is a bit low, isn't it?
most people in Bristol run similar. much better on the legs for stopping/starting/skidding. A lot used to run over 70" and then learned around 65-70" was ideal for city riding and covering more miles/proper hills outside the city.

Yeah I hear what you're saying. But for the past month, my knee pains have subsided greatly. If 48/17 causes minor pain I'll just switch to spin-to-win48/19. No biggie. 50/19 has been treating me well though.

Riding a higher ratio now n then is quite nice though, just not daily.
I don't know how a 48:19 is doable unless your cruising with risers, so spinny, 49:18 is spot on, flip to the 17t now n then
