This looks as though the cap is bottomed out on the top of the steerer, not pressing on the stem. Presumably hasty show bike assembly, but a warning to everybody, when you apply the preload the clearance between the underside of the cap and the top of the steerer closes up.
At some point in the future I might make some thin spacers which match the OD of the top cap, so you* can put an extra few millimetres between the cap and the stem if you don't want to trim your steerer to a high precision but want to avoid the diameter mismatch you'd get with most spacers which are quite thick walled.
